DD Form 1949-1 Part I LSAR Data Selection Sheet

The DD Form 1949-1 Part I is a form that was released by the U.S. Department of Defense (DoD) on August 1, 1996. The form, often mistakenly referred to as the DA Form 1949-1 Part I, is a military form mostly used within the U.S. Army. The DoD no longer supports separate instructions for the form.
